Long Term Home Health Care at St. Johnland offers participants an at-home, cost effective alternative to nursing home or other residential care. Our team customizes a plan to meet the specific needs of each individual.

Our Home Health Care participants are able to receive physical, occupational and speech therapy, nutrition counseling, personal care, nursing and social work services while still living at home. Involvement in this program can often relieve exhausted caretakers who themselves need a respite from caring for a loved one.

Each participant works with his own case manager, a compassionate and experienced registered nurse who will coordinate his case. In addition, a nursing staff member is on-call 24 hours-a-day. Our team will assist the participant and his family in acquiring a personal emergency alarm system, making certain home improvements, social work intervention, and medical or adult day care referrals, if needed.

St. Johnland's program is licensed by the New York State Department of Health.
